Implant removal — "explant" surgery — removes implants without replacing them. Reasons range from a change of preference to medical issues such as rupture, capsular contracture or symptoms a patient attributes to their implants.
What happens to the breast
After removal the breast usually looks smaller and may appear deflated or saggy, especially if implants were large or in place for many years. Some volume and shape can return over time, but many patients consider a lift to restore shape.
The capsule
The surgeon will discuss whether to remove the surrounding capsule. Routine total capsule removal is not always necessary, but may be advised in specific situations (for example certain ruptures or where pathology is a concern). This should be an individualised decision.
