- Over the muscle: easier recovery, no animation, but less cover — needs good tissue.
- Under the muscle: most cover, possibly lower capsular contracture, but more recovery and animation.
- Dual plane: the common modern default — cover up top, natural lower pole, modest animation.

How the decision is made
Your surgeon weighs your tissue thickness, degree of sagging, chest shape, implant size and lifestyle (for example, athletes who heavily use the chest muscle may prefer to avoid full submuscular placement). The plane is chosen with you as part of the overall plan.
