In submuscular and dual-plane augmentation, the surgeon releases part of the pectoralis muscle along its lower attachment so the implant can sit correctly and the breast tissue can redrape over it.
Why release is needed
Without controlled release, the muscle can sit too tightly over the implant, restrict the lower pole, or push the implant upward. Releasing the lower fibres allows the implant to settle into a natural position and the lower breast to fill out smoothly.
Tailoring the release
The amount of release defines Dual Plane I, II and III. Too little can leave a tight lower pole or double bubble; too much can over-expose the implant. Judging the right amount for your tissue is a core surgical skill.
